Dear Parent,

The Indicator 8 Parent Survey is for parents of students with IEPs in your child's district. It has statements regarding your child’s school that you are asked to rate. The ratings you select from are; very strongly disagree, strongly disagree, disagree, agree, strongly agree, or very strongly agree for each statement. There are no written responses.

Your opinions are very important to us. Your answers will be combined with those from other parents to help the state and individual school districts improve family-school partnerships in special education. No one from your school district will know how you answered the survey questions; it is anonymous.

There are three ways or options to submit your survey.

Option 1- Complete the survey you received in the U.S. Mail. You may return it by U.S. mail in the postage-paid and addressed envelope directly to Measurement Inc.

Option 2- Complete the survey online at:www.mievaluation.com/​isbe2023.htm. Your username is the seven-digit Survey ID number on the English language side of the survey you received in the mail. If you did not receive a survey contact Measurement, Inc as indicated below. *

Option 3– Complete the survey online by using the QR code. The QR code is found on the upper right-hand corner of the survey. Your username is the seven-digit Survey ID number on the English language survey side.

* If you do not have the seven-digit code, simply email or call Measurement, Inc. and they will provide it to you along with assistance in English or Spanish.

Surveys should be completed as soon as possible. All surveys received by May 15, 2023, will be included in the statewide results.
